How to Report an Error

Libman Education’s primary responsibility is to provide content that is accurate, by-the-book, and complete, leading to coding that is defensible. We are your partner providing the online training materials you depend on to ensure your understanding of complex coding challenges is up-to-date and correct. We are not perfect but we believe we should strive to be. By correcting errors quickly, we build on your essential trust in our content.

As thoughtful professionals, there is an obligation on the learner to view our content critically and ensure it is correct and applicable to their coding circumstance. As your partner for training materials related to medical record coding, it is our obligation to ensure our materials are as accurate as possible. When errors are discovered in our materials, we act quickly.

The corrections process:

• First, we determine if the content in question contains an error. Coding is a complex process of turning medical information into a structured system of defined diagnoses and procedures. There are guidelines that define how codes may be applied and reputable advice to consider. What may appear to be an error may be a simple misunderstanding. In those cases we take the time to explain why we believe our content is correct.
• Knowledge is power. Skilled coders can disagree on how best to represent the patient’s story accurately leading to different approaches to the coding. A disagreement on how to code a condition or procedure does not necessarily mean there is an error in the training materials. Coding often contains ‘grey areas’ where more than one right answer can exist. In these cases we lay out the conflicting information and advice allowing the organization to make the best choice for them.
• Simple corrections to our training materials are posted immediately to the course and made available to all learners.
• Complex corrections, those that significantly change how to approach a coding challenge, are sent to current learners who have already completed a course or are currently working in the course. As appropriate, this information may also be shared with our newsletter readers.
